Hypertensive Disorders of Pregnancy

Hypertensive disorders of pregnancy refer to a spectrum of conditions caused by high blood pressure in the mother during pregnancy. Hypertensive disorders affect 5-8% percent of women. There are four general categories: chronic hypertension, gestational hypertension, preeclampsia-eclampsia, and preeclampsia superimposed on chronic hypertension. Chronic hypertension and gestational hypertension refer to high blood pressure present before or early in the pregnancy (chronic), or develops and persists during pregnancy (gestational), but goes away after birth. Many women with high blood pressure have normal pregnancies, but high blood pressure can cause low birth weight, early delivery, and damage to the mother's organs. The more elevated the blood pressure, the higher the risk for complications. Treatment of high blood pressure includes medication and possibly diet changes and bed rest. High blood pressure is also a major factor in the development of preeclampsia-eclampsia and preeclampsia-eclampsia superimposed on chronic hypertension. Preeclampsia is a serious condition caused by high blood pressure which damages the pregnant woman's kidneys and potentially the liver and brain. Preeclampsia is detected by high blood pressure and presence of protein in the urine. Treatment of preeclampsia depends on the timing during the pregnancy. Left untreated, preeclampsia may progress to eclampsia causing the woman to have seizures. Eclampsia is life threatening to both mother and baby and delivery of the baby must be performed immediately. A woman should have regular checkups during the pregnancy so that high blood pressure can be detected early. (See also Eclampsia, Gestational hypertension, and Preeclampsia.)
